VERIFICATION STATEMENT

For purposes of legal conformity, for the protection of Excalibur, and in the best interests of your child, Excalibur requires that any party who has a legal interest in your child’s education (particularly a non-custodial parent) must verify the information below and that given on the Church School Enrollment Form (provided at the interview) by their signature. Please carefully read the following statements and choose the appropriate statement that reflects your child/children’s parentage. All parties who have an interest will be required to sign a form acknowledging the appropriate statement of those given below in the presence of an Excalibur witness or a notary public.

Two parent home:

• We verify that we have provided the information set forth on the accompanying Excalibur Christian School Enrollment Application and that it is true and complete to the best of our knowledge.  We further verify that we are the parents of the child(ren) who is (are)applying for enrollment in Excalibur and that there is no other parent or legal custodian from whom the child must have permission.

Single-parent home or parent/step-parent custodial parents (Check this statement if there is not a non-custodial parent or guardian whose agreement might be required by law.):

• I verify that I have provided the information set forth on the accompanying Excalibur Christian School Enrollment Application and that it is true and complete to the best of my knowledge.  I further verify that I am the parent of the child(ren) who is (are) applying for enrollment in Excalibur and that there is no other parent or legal custodian from whom the child must have permission.
  

Single-parent home or parent/step-parent custodial parents with non-custodial parent or guardian whose agreement is or may be required by law (Check this statement if there is a non-custodial parent or guardian whose agreement might be required by law.):

• I verify that I have provided the information set forth on the accompanying Excalibur Christian School Enrollment Application and that it is true and complete to the best of my knowledge.  I further verify that I am the custodial parent of the child(ren) who is (are) applying for enrollment in Excalibur.

and

• I verify that the information set forth on the accompanying Church School Enrollment Form(s) is true and complete to the best of my knowledge.  I further verify that I am the non-custodial parent of the child(ren) who is (are) applying for enrollment in Excalibur and that I have read all of the enrollment documents and policies and approve of his/her/their enrollment. (A non-custodial parent who does not live in the immediate area must have his/her signature notarized and attach a notary acknowledgement to this form.)
